Acura MDX problems and known issues

By CarWhere Research · Updated

The Acura MDX issues with the biggest consequences are recalls: 2001–2002 transmission gear failure, Takata air bag inflators on 2003–2006 models and the 2016–2020 connecting rod bearing recall (23V751). The 2022-on fourth generation adds Type S stalling software and a brake pedal pin fix. Run any MDX VIN for open campaigns before buying.

CarWhere lists 19 documented MDX issues, including 19 NHTSA safety recalls, each with affected years, powertrains, symptoms, pre-purchase checks and a source. Recall entries come from NHTSA's recall dataset on the DOT open-data portal, which may not list every campaign, so run the specific VIN through NHTSA's recall lookup.

Recall 25V031 covers 2022–2025 MDX Type S models, 2021–2025 TLX Type S models and 2023–2025 Honda Pilots. A software error in the fuel-injection control unit (FI-ECU) can make the engine stall or lose power while driving. Dealers reprogram the FI-ECU for free; owner letters went out March 17, 2025. The 294,612-vehicle count covers all three models.

The brake pedal pivot pin on certain 2023–2025 MDX, 2021–2025 TLX and 2023–2025 Honda Pilot models may not have been secured, so the pedal can shift out of position. That puts the vehicles out of compliance with federal brake and lighting standards (FMVSS 135 and 108). Dealers inspect and, if needed, replace the pedal assembly. The 259,033 total spans all three models.

Symptoms

  • Brake pedal feels loose or sits at an odd angle
  • Brake lights stay on or fail to come on

3.5L V6 with 5-speed automatic

In April 2004 American Honda recalled 2001–2002 MDX models, along with certain Odyssey and Pilot models with V6 engines and five-speed automatics, because the transmission’s secondary-shaft second gear could run short of lubrication. Heat build-up can chip or break gear teeth and, in rare cases, lock the transmission. Dealers modify the cooler return line to boost lubrication and replace the transmission if the gear shows heat discoloration. Frequently asked questions

What are the known Acura MDX transmission problems?

The documented MDX transmission issue is recall 04V176: on 2001–2002 models the five-speed automatic’s second gear could run short of oil, overheat and chip or break. Dealers modified the cooler line and replaced transmissions that showed heat damage. Confirm the repair by VIN before buying an early MDX.

Which MDX years have the engine connecting rod recall?

Recall 23V751 covers certain 2016–2020 MDX models, along with 2015–2020 TLX and some Honda V6 models. A connecting rod bearing can wear and seize. Dealers inspect the engine and repair or replace it free of charge, so ask for the paperwork.

Are older MDX models affected by Takata air bag recalls?

Yes. 2003–2006 MDX models are in the Takata driver (15V320) and passenger (16V344, 17V029) inflator recalls, with follow-up campaigns for replacement parts. 2001–2002 models are in separate non-Takata driver inflator recalls (20V026, 20V027). All repairs are free.

What recalls affect the 2022 and newer MDX?

The fourth-generation MDX has a fuel-injection software recall for the Type S (25V031), a brake pedal pivot pin recall for 2023–2025 models (25V391), a 2025 touchscreen and camera recall (25V032) and the 2026 seat weight sensor recall (26V332).

Is there a 2021 Acura MDX?

No 2021 MDX appears in these recall records: campaigns that span both generations list 2017–2020 and then 2022-on model years. The fourth-generation MDX arrived for the 2022 model year.

How do I check an MDX for open recalls?

Enter the 17-digit VIN at NHTSA’s recall lookup and at recalls.acura.com. Recall repairs are free at any Acura dealer no matter how old the vehicle is or who owns it.
